CONDITIONS OF USE

A Membership Waiver must be completed by every participant prior to facility use.

An Evolution Health & Fitness membership is an exclusive entity that cannot be shared or transferred between parties without the permission of management.

Members are required to scan their membership tag or key fob upon each visit. Evolution Health & Fitness reserves the right to request photo identification where member identification is questionable.

Any membership may be revoked or suspended by management for violation of Evolution Health & Fitness’ membership or facility policies without notice or refund. No leeway circumstances include:

Failure to pay any amount owing Evolution Health & Fitness when due
Use of facility or any equipment in a dangerous or unapproved manner
Disrespect for staff or other patrons
Shared use of membership tag or key fob
Providing false or misleading information
Theft or vandalism

CLIENT ETIQUETTE REQUIREMENTS

AT ALL TIMES

Self-monitor an do not attend our facility if you display any signs of illness
Change into Indoor Specific exercise shoes
Scan-in or Check-in with Front Desk staff upon arrival
Wash and sanitize your hands regularly
Arrive prepared to minimize over-crowding of change rooms
Complimentary lockers should be used to secure valuables
Personal Sweat Towel and Spray Cloth provided

- Sweat Towel to cover upholstery and personal hygiene
- Spray Cloth to be used with Spray Bottle Stations to sanitize equipment after use
- Both towels to be returned to Laundry Basket at Front Desk after workout

Avoid peak demand times 9-11am & 5-7pm whenever possible
Limit exercise routine to 60 minutes during high demand
Return all equipment to proper storage locations after use

DURING CARDLOCK SPECIFIC HOURS

Under 18 are permitted cardlock privileges but are limited to 6am to 8pm
Key Fobs are personal and exclusive and may not be shared
Every entrant must scan their individual Key Fob upon entry
- NO multiple entry on a single key fob scan, even if both members are permitted clients
Keep a mobile phone with you at all times for emergencies
- Panic button activates facility alarm in cases of serious distress
All emergency doors are alarm activated during unstaffed hours
